Provide School Supplies for a Foster Child

Act Together Ministries is looking for people in the community to provide school supplies for local foster children.

Plan to spend $85-100:

You will receive an e-mail about your assigned child. You will be given the child’s interests/preferences, grade level, and school supply list.

Financial donations in any amount can also be made online. Make a Donation Today.

Backpacks and school supplies will need to be dropped off by Friday, July 21st. Drop off locations in Roswell, Alpharetta and Woodstock.

By submitting this application, I agree to the following terms: As a condition of my being allowed to sponsor a child through Act Together Ministries, Inc., a Georgia nonprofit corporation (“Act Together”), I hereby voluntarily and absolutely release and discharge Act Together, and its officers, directors, agents and employees, successors and assigns from any and all loss or damages or actions or causes of action of any kind and character that I may suffer as a result of my sponsoring a child for any Act Together programs; whether or not such injuries or damages are caused by the negligence (active or passive) of any of the entities or individual named above. I agree to abide by the rules and regulations governing Act Together and to obey any instructions given by Act Together concerning the program in which I am volunteering as a sponsor. I realize and acknowledge that some of the activities/events described above may involve my entry into businesses or homes where I was instructed to deposit/drop off gifts purchased for the sponsored child. I hereby acknowledge the risk of entering the homes of recipient persons/families and assume the risk of same. I will indemnify and hold harmless Act Together and its directors, officers, agents, servants or employees, other volunteers/sponsors, successors and assigns from any and all claims or causes of action by myself or by any other person or entity, and under no circumstances will I or my successor, heirs, assigns, or personal representative present any claims against said organization and said persons for any injury, damages, cause of action, of any kind or character caused by any act of negligence by Act Together or any third party while I am volunteering with or sponsoring a child with Act Together. I agree that in the event of an injury to myself as result of my volunteering with Act Together, whether or not caused by the negligence (active or passive) of Act Together, or any of its officers, directors, agents or employees, recourse for the payment of any hospital, medical, dental, or related costs and expenses will be paid either by me or my spouse, or parent, accident, hospital or medical insurance, or any available benefit plan of mine or my spouse or my parent (if applicable). I authorize the making of photographs, motion pictures, videotapes, recordings, or other memorializing of said event and my participation therein, and the publication or other use thereof. I waive any right to compensation therefore or any right that I otherwise might have to limit or control such making or use. I warrant and represent that I am eighteen years of age or over, and am fully aware of and understand the terms and legal consequences of the signing of this Waiver and Release. I intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law. This agreement shall be governed by the laws of the State of Georgia. Jurisdiction and Venue for any matter concerning this agreement shall be arbitrated according to the rules of the American Arbitration Association and held in Atlanta, Georgia.
